When barriers to accessing services make it difficult to get the support you need, having someone to help navigate the system can make a meaningful difference. Doorways Community Connections supports youth ages 15-25 and their families in accessing services and resources that promote well-being and success.
The program helps youth and families overcome barriers, connect with appropriate supports, and access services that meet their unique needs and goals.
Through intensive counselling and case management, Psychotherapists and Service Coordinators work closely with youth and their families to address challenges related to mental health, healthcare, education, housing, and other community supports. Services are tailored to each individual’s circumstances and focus on strengthening connections to resources, improving access to care, and building the skills and supports needed to navigate complex systems. By providing personalized guidance and coordinated support, the program helps youth and families achieve greater stability, well-being, and connection to their communities.
