For individuals ages 16 and older with a suspected or confirmed dual diagnosis, developmental disability, or complex behavioural needs who are experiencing challenges that place them at risk of crisis, instability, or service disruption.
LCSN helps individuals and families build stability, strengthen supports, and navigate complex situations so they can remain safely connected to their communities and avoid more intensive crisis services whenever possible.
Through a network of community partners, LCSN provides time-limited crisis, prevention, and transitional supports tailored to each person’s unique needs. Services may include community outreach, assessment and stabilization, individualized support planning, residential safe beds, day programming, and coordination with family members and service providers. The Network works collaboratively across sectors to address complex situations in homes, hospitals, shelters, placements, and other community settings. By connecting individuals and families to the right resources, securing needed supports, and facilitating access to developmental services, LCSN helps create pathways toward greater stability, well-being, and long-term success.
