Complex Special Needs Children Program (CSN)

Complex Special Needs (CSN) supports children, youth and adults with significant unmet needs through individualized planning, service coordination, and specialized supports.

When your child has complex needs that cannot be adequately supported through existing community services, finding the right help can feel overwhelming. Complex Special Needs (CSN) supports children, youth, adults and families in Toronto whose needs require specialized planning and coordinated supports.

The program helps get access appropriate services, address gaps in support, and achieve greater stability at home and in their communities.

Once approved through the Ministry of Children, Community and Social Services (MCCSS), families work with a Case Facilitator to develop an individualized service plan. Working alongside families and community partners, the program coordinates specialized supports and resources that reflect each child’s unique strengths, needs, and goals, helping them access the right level of care and support.
