Community Support Services

Community Support Services helps adults with developmental disabilities and dual diagnoses build life skills, maintain housing, increase independence, and connect to community supports.

When finding and maintaining stable housing feels challenging, having the right support can make a meaningful difference. Community Support Services supports adults with developmental disabilities and/or dual diagnoses who are experiencing housing instability, are at risk of homelessness, accessing shelters, or transitioning from hospital to community living.

The program helps individuals build the skills, confidence, and supports needed to live safely, independently, and successfully in their communities.

Through individualized assessments, life skills development, and community-based supports, participants work toward goals related to housing, daily living, financial literacy, employment, relationships, social skills, health, and community participation. Staff help identify barriers, develop personalized plans, and connect individuals to services and resources that support long-term stability. Services are provided through one-to-one support, group programs, and community partnerships, helping participants strengthen independence, expand their support networks, and improve their quality of life.
